There are a couple different ways to submit an application to me:

1. Print out the application and fill it out. Then scan it and email it back to me.
If you do this you can either mail the application fee to me in the mail or send via paypal

2. Print out the application and fill it out. Then mail it along with payment to me.
If you choose to mail it please call and let me know that you have mailed it so I can keep my eyes open for it.

3. Print out the application and fill it out. If you would like to fax it to me call me to see what fax number I am at for you to send it to.
